Dynamo Swim School Policies and Release

Please read and sign the following. Please initial each paragraph

___ REGISTRATION FEE: There is a non-refundable registration fee of $25/family/year, renewable

each January, or $5/student/session payable at time of registration if registering in person. If registering on-line the registration fee is due on the first day of class. Your space will be held but your registration is not complete until the registration fee paid.

___ REFUND POLICY: NO REFUNDS. We hire instructors based on our registration.

___ MAKE-UP POLICY: WE DO NOT OFFER MAKE-UP CLASSES for illness or conflicts with other activities.

___ CANCELLATION POLICY: To cancel a class you must withdraw at least TEN DAYS prior to the beginning of the class. If you withdraw from class, you will not receive a refund. You will receive a credit for the amount you paid for the class less a 15% cancellation fee. No credit will be issued if you withdraw from class less than TEN DAYS prior to beginning of class.

___ SCHEDULE CHANGE POLICY: Schedule changes are subject to class/session availability. All changes must be made at least TEN DAYS prior to scheduled session. All changes are subject to an $8.00 administrative fee. Please choose classes carefully.

___ ILLNESS POLICY: Credits will be issued ONLY for extended illness (three or more consecutive lessons) with a note from your doctor. The credit will be $9.50 a lesson for preschool, gradeschool or adult $8.50 a lesson for toddlers, and $18 for stroke clinic.

___ CREDIT POLICY: Credits will not be issued for conflicts with other activities or vacations. Credits will be issued for extended illness only (see illness policy above).

___ USE OF SWIM DIAPERS: ALL students under the age of five (5) years are required to wear a reusable swim diaper. These must have tight fitting elastic around the waist and legs. Disposable swim diapers are NOT allowed. Students under the age of five will not be allowed in the pool without the appropriate swim diaper. NO EXCEPTIONS. Swim diapers may be purchased at the swimshops located inside of Dynamo. Swim diapers should be purchased before the first day of class.

___ POOL CLOSINGS: When a child vomits or has a bowel movement in the pool, the pool must be closed for sanitation. Should this occur, space permitting, we will move to the competition pool at the Alpharetta facility. If not, we will try to schedule a make-up. Make-up lessons will be scheduled on Saturdays. If a make-up cannot be scheduled a credit for $9.50 will be issued for each preschool, gradeschool, or adult student, $8.50 for toddlers, and $18.00 for stroke clinic students. If a make-up class is scheduled, no credits will be issued.

___ POOL CLOSING FEE: If your child vomits or has a bowel movement in the pool thereby causing the pool to be closed, you will be assessed a fee of $200.00. Your child will not be allowed to participate in class until the fee is paid.

___ SEATING FOR PARENTS AND SIBLINGS: Parents and siblings of swim lesson students that wish to stay in the building during lessons must sit in the bleachers near the competition pool. You may not sit or stand on the pool deck surrounding the teaching pool. This includes the area around the front counter and the lifeguard stand. The last five minutes of each class and the last class of each session you may sit in a designated area and watch the progress of your child.

RELEASE AND INDEMNIFICATION For and in consideration of the benefits to me from the use by me and/or my child(ren) of the facilities of Dynamo Parents Club, Inc. (Dynamo Swim Center) and other good and valuable consideration, the undersigned, individually and of his /her heirs and personal representatives, hereby release Dynamo Parents Club, Inc. directors, officers, agents, swim team members and employees from any and all claims of any kind or nature whatsoever, arising out of the use by me and/or my child(ren) of said facilities. The undersigned, individually and for his/her heirs and personal representatives, further agree to indemnify, defend and forever hold harmless Dynamo Parents Club, Inc. and their directors, officers, agents, swim team members and employees from any and all liability or loss whatsoever, (including any cost of defending claims) arising out of said use of said facilities.
