Dynamo SwimAmerica Policies and Release

 

     Please read and sign the following.  Please initial each paragraph:

 

____ON-LINE REGISTRATION: No multiple child discounts will be given if registering on-line.  Credits may not be used if registering on-line.

 

____MAKE-UP POLICY: WE DO NOT OFFER MAKE-UPS CLASSES.

 

____REFUND POLICY: NO REFUNDS. We hire instructors based on our registration.

 

____WITHDRAWAL POLICY: If you decide that you cannot participate in a class you will not receive a refund, but a credit for the amount you paid for that particular class less a 10% administrative fee.  To receive this credit, you must withdraw from the class at least two weeks prior to the beginning of that class.  No credit will be issued if you withdraw from a class less than two weeks prior to the start of that class.

 

____CREDIT POLICY: Credits will be issued ONLY** for extended illness (three or more consecutive lessons) with a note from your doctor.  Credits will not be issued for conflicts with other activities or vacations.

 

____CHANGE POLICY: Schedule changes are subject to class/session availability.  All changes must be made at least two weeks prior to scheduled session.  Please choose classes carefully.

 

____USE OF SWIM DIAPERS: ALL students under five (5) years of age are required to wear an approved swim diaper.  These must have tight fitting elastic legs and waist.  Disposable swim diapers are NOT allowed.  Students under five years of age will not be allowed in the pool without the approved diaper.  These diapers may be purchased at the Friends Unlimited store located inside of Dynamo, Peek-a-Boo children’s store in Abbotts Kids Village.  If your child weighs 40+ lbs., you will find a larger selection on the Internet by searching under “swim diapers.” 

 

____POOL CLOSING FEE: If your child vomits or has a bowel movement in the pool thereby causing the pool to be closed, you will be assessed a fee of $200.00.  Your child will not be allowed to participate in class until the fee is paid.

 

____SEATING FOR PARENTS AND SIBLINGS: Parents and siblings of swim lesson students that wish to stay in the building during lessons must sit in the bleachers near the competition pool.  You may not sit or stand on the pool deck surrounding the teaching pool.  This includes the area around the front counter and the lifeguard stand. The last five minutes of each class and the last class of each session you may sit in a designated area near the pool and watch the progress of your child.   

 

____** If your child has been ill or has had a fever within the last 24 hours, please do not bring him to class.  When a child vomits or has a bowel movement in the pool, the pool must be closed.  When this occurs, we will do a water safety class for the first incidence in a session.  Part of this class may take place in the competition pool.  If this occurs a second time a credit for $9.50 for preschool, gradeschool, and adult classes will be issued to each student enrolled in that session.  For toddlers, the credit will be $8.50.  Only one credit per session will be issued. 

 

RELEASE: 

1.  We do everything possible at Dynamo to insure the safety of your child.  We have certified lifeguards on duty.  Safety rules are enforced. While our first priority is your child’s safety, we must inform you that swim lessons are not risk free.

 

2.  As a participant or as a parent or legal guardian of a participant in the Dynamo SwimAmerica program, I am fully aware that swimming can be strenuous, hazardous, and difficult.  I acknowledge and freely accept the risks and hazards associated with the participation in swimming.  I agree to hold Dynamo Swim Club, its officers and agents free and harmless for any injuries of damages arising by reason of participation in this program.
