Dynamo Swim School Policies and Release

 

 

Please read and sign the following.  Please initial each paragraph:

 

 

 

____REGISTRATION FEE: There is a non-refundable registration fee.  There are two* payment options:

1.        $50/Family: The family fee may be paid at any time during year.  However, it covers classes from January – December of 2008.   It expires December 31 each year.  This option is not available if registering online.

2.        Per Session: $10/child/session   

*Please decide which option is best for your family.          

        

 

____REFUND POLICY: NO REFUNDS.   We hire instructors based on our registration

 

____MAKE-UP POLICY: WE DO NOT OFFER MAKE-UPS for illness or conflicts with other activities.

 

____CANCELLATION POLICY:  If you withdraw from class, you will not receive a refund.  You will receive a Dynamo Swim School credit for the amount you paid for the class (registration fee not included) less the following cancellation fees:

 

 

·          10+ days prior to session – 15% 

·          1-4 days prior to session – 50%

·          5-9 days prior to session – 25%

·          0 days – No credit will be issued

 

  ____SCHEDULE CHANGE POLICY: Schedule changes are subject to class/session availability.  Changes must be within the current season.  Please choose classes carefully.   Changes are subject to the following administrative fees:

                                    

·          10+ days prior to session – $9.00 

·          1-4 days prior to session – $25.00

·          5-9 days prior to session – $12.00

·          0 days – No changes allowed.

 

____ILLNESS POLICY: If your child is absent from class due to illness for THREE CONSECUTIVE CLASSES a credit will be issued when a note from his doctor is presented.  No credit will be issued without a note from the doctor.  The note must have the dates that the child was not allowed to be in class.  The credit will be $9.00/class for swim lessons, $13.00/class for adult swim lessons, and $16.00 for stroke clinic.

 

____CREDIT POLICY: Credits will be issued for extended illness (see illness policy above) and pool closures only. Credits will not be issued for conflicts with other activities or vacations.

 

____CREDITS: Dynamo Swim School credits may be applied toward future swim lessons, group or private. They may be used towards other non-competitive programs (recreational swim pass, aerobics, etc.).  Dynamo Swim School credits may not be applied towards swim team dues or entry fund, unless that child is moving from lessons to swim team.  Credits slips must be presented in order to be redeemed.  Credits will not be replaced if lost or stolen.  Credits are not transferable.  Credits are NOT redeemable for cash.

 

____USE OF SWIM DIAPERS: ALL students under the age of five (5) years are required to wear a reusable swim diaper.  These must have tight fitting elastic legs and waist.  Disposable swim diapers are NOT allowed.  Students under the age of five will not be allowed in the pool without the approved diaper.  NO EXCEPTIONS.  Swim diapers may be purchased at the swim shops located inside of Dynamo.   Swim diapers should be purchased before the first day of class. 

 

____ POOL CLOSINGS: The teaching pool will be closed for the following reasons:

1.        When a child vomits or has a bowel movement in the pool, the pool must be closed for sanitation. 

2.        Inclement weather - Please call the facility at which you are enrolled.

3.        If conditions in the pool make it unsafe for students to be in the pool, the pool will be closed.

 

 

____When the pool is closed for any of the above reasons a credit will be issued.   The credit will be $11.00/class for swim lessons, $16.00 for adult swim lessons, and $20.00 for stroke clinic.  Please refer to CREDITS paragraph above.

 

____POOL CLOSING FEE: If your child vomits or has a bowel movement in the pool thereby causing the pool to be closed, you will be assessed a fee of $200.00.  Your child will not be allowed to participate in class until the fee is paid.

 

____SEATING FOR PARENTS AND SIBLINGS: Parents and siblings of swim lesson students that wish to stay in the building during lessons must sit in the bleachers near the competition pool at the Alpharetta facility.   You may not sit or stand on the pool deck surrounding the teaching pool.  This includes the area around the front counter, the water fountain, and the swim shop.    At the Chamblee facility you must sit in bleachers in the balcony.  You may not sit or stand on the stairway, or block it in anyway, at the Chamblee facility. The last five minutes of each class and the last class of each session you may sit in a designated area and watch the progress of your child.

 

RELEASE AND INDEMNIFICATION:  For and in consideration of the benefits to me from the use by me and/or my child(ren) of the facilities of Dynamo Parents Club, Inc.(Dynamo Swim Center) and other good and valuable consideration, the undersigned, individually and of his/her heirs and personal representatives, hereby release Dynamo Parents Club, Inc. directors, officers, agents, swim team members and employees from any and all claims of any kind or nature whatsoever, arising out of the use by me and/or my child(ren) of said facilities.  The undersigned, individually and for his/her heirs and personal representatives, further agree to indemnify, defend and forever hold harmless Dynamo Parents Club, Inc. and their directors, officers, agents, swim team members and employees from any and all liability or loss whatever, (including any cost of defending claims) arising out of said use of said facilities.
